First Visit

IMG_0917-1Your initial appointment will begin with a thorough medical and dental history in addition to an oral exam.  

Typically, Dr. Hirschfield Louik, Dr. Chory or Dr. Racic will spend several minutes talking with you outside the treatment area to begin to get to know you.  We want to understand your previous dental experiences and give you the opportunity to share your thoughts and concerns with us.  In our opinion, an informed and involved patient is more likely to achieve a higher level of healthcare.  The oral exam includes a hard tissue (teeth) and soft tissue (gums) examination as well as an important screen for oral cancer.  A preliminary consultation explaining your diagnosis and treatment options will be discussed at the end of your first visit.  Occasionally, treatment can be performed the same day as the consolation.  However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on a subsequent day.

IMG_0959-1We are a family friendly office.  Unique to our office is a separate children’s play area allowing parents the opportunity to bring their young children with them to the office.  Siblings can also be entertained here while their brothers or sisters receive treatment.  Our adult patients appreciate the separate quiet reception area reserved just for them.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.
  • A list of medications you are presently taking.
  • Applicable insurance benefit information
  • A completed health history if not completed online

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If you have recent x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.  We have state of the art digital technology for X-rays which decreases your exposure to radiation and increases your comfort.
